Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online regsitration form fill out a Health Questionnaire.

You may also need to email the practice with a picture of your photo identification and proof of address such as bank statement, recent utility bill showing your name and current address.

When normal service resumes, you will be asked to bring the hard copies into the practice.

We welcome new patients to the surgery who reside within the catchment area. You can check if you are within our catchment area by entering your postcode below.

 
 

Registering with the Practice

The practice has an open list and is happy to accept registration requests from anyone living within the practice area.

You will need to come into the surgery to register with us, but to save time and speed up the registration process you may wish to download the appropriate practice information forms needed to complete your registration by clicking on the link below. This form can then be printed and filled out by yourself prior to your visit to the surgery.

On visiting the surgery you will need to bring proof of your identity and know your NHS number, which is required on the NHS registration form which we will complete with you.

Named GP

Whilst you are registered with the practice, every patient has a named GP who is responsible for overseeing their clinical care, they are known as your 'Usual GP'.  If you are unsure who your 'Usual GP' is please ask.
